   1. I consent to the church holding the information below on the basis described above.

   2. I give permission for my child to attend the outing and to participate fully.

   3. I give permission for first aid to be administered and for medical treatment to be given by a suitably qualified medical practitioner. Should my child require emergency hospital treatment, I authorise a church youth leader to sign on my behalf any written form of consent required by the hospital if I cannot be contacted. However, I understand that every effort will be made to contact me as soon as possible.

   4. I confirm that the details I am giving on this form are correct to the best of my knowledge. I will inform the church of any changes.
